OPERATORS <br /> SHOULD READ THE FOLLOWING PAGES CAREFULLY IN ORDER TO ASSURE COMPLIANCE <br /> WITH THE TERMS OF THE PERMIT AND APPLICABLE RULES AND REGULATIONS. If a <br /> Possible Violation is indicated, you will be notified under separate cover as to when the Mined Land <br /> Reclamation Board will consider possible enforcement action. <br /> INSPECTION TOPIC: Hydrologic Balance <br /> PROBLEM/POSSIBLE VIOLATION: Problem: The groundwater monitoring program required by AM02 and <br /> approved on March 18, 2013 has not yet been implemented, and in fact, the required monitoring wells have not <br /> yet been installed. <br /> CORRECTIVE ACTIONS:The new groundwater monitoring wells required by the monitoring plan as approved in <br /> AM02 will be installed no later than 10/31/14, and groundwater level data collection from all monitoring points <br /> will commence immediately thereafter as outlined in the approved plan. <br /> CORRECTIVE ACTION DUE DATE: 10/31/14 <br /> INSPECTION TOPIC: Compliance With Mine Plan <br /> PROBLEM/POSSIBLE VIOLATION: Problem: The current mine plan needs to be updated and clarified pursuant to <br /> C.R.S. 34-32.5-112 (1)(c)(VI). The operator must provide sufficient information to describe or identify how the <br /> operator intends to conduct the operation. <br /> Evidence of concrete and asphalt recycling activities was observed on the site. To date these recycling activities <br /> have not been approved as part of the mining plan for this permit. In fact, these activities were specifically <br /> removed from the AM01 mining plan during the adequacy review process because these activities would <br /> require approval by Weld County. <br /> A technical revision shall also be submitted to reflect the changes to the site configuration due to recent and <br /> ongoing oil and gas drilling activities such as - plugged wells, new wells, gathering lines, etc.. <br /> CORRECTIVE ACTIONS:The operator shall submit a Technical Revision, with the required $216 revision fee, to <br /> update and clarify the current approved mine plan to reflect existing and proposed activities by the corrective <br /> Page 1 of 5 <br />
