Box 841 <br /> Canon City, CO 81215 <br /> RE: Fremont Paving&Redi-Mix,Inc.,Evans#2 Pit,Permit No.M-2000-041; <br /> Amendment Approval,Revision No.AM-01 <br /> Dear Mr. Ary: <br /> On July 31, 2014 the Division of Reclamation, Mining and Safety (Division) approved the Amendment <br /> application submitted to the Division on July 30,2013,addressing the following: <br /> Change final reclamation from unlined Evans Lake to lined water storage reservoir with wetlands <br /> and open space for recreation. <br /> The terms of the Amendment No. 1 approved by the Division are hereby incorporated into Permit No. M- <br /> 2000-041. All other conditions and requirements of Permit No.M-2000-041 remain in full force and effect. <br /> The estimated liability amount of$1,994,600 for Phases IA and 113 (currently disturbed areas) exceeds the <br /> $568,483 performance bond currently held. Please submit additional bond in the amount of$1,426,117 within <br /> 60 days of the date of this letter. The revision will not be final until the bond is approved by the Division. <br /> The Division has calculated the bond for Phase 2 to be an additional $100,400. This additional bond must be <br /> submitted prior to disturbing any of the Phase 2 Evans#2 Pit area,but no later than 365 days from the date of <br /> this letter.
