Director <br />2489 CR 15 <br />South Fork, CO 81154 <br />Re: O'Rourke Pit, South Fork Pit, File M- 2014 -005, Technical Adequacy Review Letter <br />Dear Mr. O'Rourke, <br />The Division has been reviewing and discussing with you various issues associated with the Construction <br />Materials 110c permit application you submitted, as named above. This letter is intended to communicate <br />to you the remaining items that are still needing clarification or correcting, before the application can be <br />considered adequate for approval. <br />Please respond to each of the following items, and label them as shown below, to facilitate the Division's <br />review. <br />Exhibit E - Mining Plan Map <br />Please revise the mining plan map to include a better delineation of the area to be mined, including an <br />indication of the direction or phases of the mining operation. If certain areas are not to be mined, the map <br />should reflect that. The map should show onsite roads, and processing and stockpiling areas. It should <br />also show all significant structures on or near the proposed permitted area, including perimeter fences. <br />Please ensure that the map is signed and dated. <br />Exhibit E - Reclamation Mao <br />Please provide a revised reclamation map, showing the site as it is expected to appear after all mining and <br />reclamation have been completed. If certain areas are not to be mined, this should be reflected. It should <br />show all roads and other mining - related features that will remain after reclamation. Please ensure that it <br />includes sufficient topographic information to clearly represent the rate and direction of all sloped <br />surfaces, and is depicted at an appropriate scale. Please ensure that the map is signed and dated. <br />Proof of Publishing Legal Notice <br />Please provide proof of publishing the legal notice in a newspaper of general circulation within the county <br />where the proposed gravel operation is location. Proof of publication may be provided in the form of the <br />newspaper page on which the notice appears, or by a notarized affidavit from the publisher. The language <br />to be used in the notice is contained in the permit application packet. Please note that if any substantive <br />error appears in the notice it may have to be republished.
