Reclamation Permit Application Package
Dear Mr. Keller,
On December 10, 2013 the Division of Reclamation, Mining and Safety (Division) received your
112 Construction Materials Reclamation Permit Application package for the Limon Pit, File No.
M- 2013 -080. Preliminary review of the information received determined that the following
items must be received before the Division can consider your application as being submitted and
technical review can begin:
Application Form:
The Applicant did not complete the General Description of item No. 9 on Page 2 of the
Application Form. Please submit a replacement page with this information included; a
replacement page has been enclosed with this letter.
2. The Applicant did not include Page 8 of the Application Form. Please submit an original
copy the Certification Page and ensure sure that all signatures are completed in blue ink; a
replacement page has been enclosed with this letter.
6.4.17 Exhibit Q — Proof of Mailing of Notices to Board of County Commissioners and Soil
Conservation District
3. The Applicant provided a copy of a date - stamped receipt from the Lincoln County Clerk and
Recorder Office for Exhibit Q. In addition, you must provide proof of notice to the Lincoln
County Commissioners. Proof of notice may be in the form of a return receipt of a certified
mailing or a date - stamped copy of the notice acknowledging receipt by the appropriate
Board.
Your application will not be considered submitted until the information listed above is received
and found sufficient to begin our review. A decision date will be established 90 days from the
date of receipt of all of the requested information. Additionally, if you have already published
notice you will need to republish notice, after the Division considers the application submitted.
This notice must be published once a week for four (4) consecutive weeks, starting within ten
(10) days of the date your application is considered submitted. We will notify you when you
