The spill <br />occurred at approximately 4:30 AM on Wednesday October 30, 2013. <br />Notice Requirements (3.1.13(2)): <br />(a) Henderson Mine, M -1977 -342, Miguel Hamarat <br />(b) (303) 569 -3221 Ext. 1233 <br />(c) 4:30 AM, October 30, 2013 <br />(d) Diesel Fuel <br />(e) An estimated a half a gallon of diesel fuel was released into No Name Creek when a <br />snowplow truck ran into the creek. Diesel fuel leaked from the tank cap. The leak <br />lasted for approximately 2 hours. The leak did not leave the permit area. <br />(f) Absorbent booms were placed in the creek and around the tank.
