As required by Rule 3.1.13 — Spill Reporting
Mr. Miguel Hamarat notified the Division of a spill of diesel fuel on October 30, 2013. The spill
occurred at approximately 4:30 AM on Wednesday October 30, 2013.

(e) An estimated a half a gallon of diesel fuel was released into No Name Creek when a
snowplow truck ran into the creek. Diesel fuel leaked from the tank cap. The leak
lasted for approximately 2 hours. The leak did not leave the permit area.
(f) Absorbent booms were placed in the creek and around the tank. Additional clean up
will be conducted following removal of the snow plow.
