PERMIT #: M- 1985 -008 <br />INSPECTOR'S INITIALS: TOD <br />INSPECTION DATE: July 19, 2013 <br />OBSERVATIONS <br />This was a normal routine monitoring inspection conducted by Tyler O'Donnell of the Division of Reclamation, Mining <br />and Safety (Division). Phillip Crum, the mine operator's representative, was present during the inspection. The Leone - <br />Gardner Gravel Pit is located approximately 3/4 miles southeast of Gardner Colorado. The Leone - Gardner Gravel Pit is <br />permitted as a sand and gravel pit. The approved permit area is 9.9 acres. The approved post- mining land use is <br />rangeland. <br />At the time of the inspection it was cloudy. The ground was dry. No mining equipment was operational or present at <br />the time of the inspection. <br />Backfilling and Grading: <br />The site has been backfilled and graded with slopes 3H:1V or flatter. All slopes appeared to be structurally stable. <br />Financial Warranty: <br />The reclamation cost estimate for this pit was updated with this inspection. It was found that the current bond amount <br />is adequate to cover the cost of reclamation. <br />Hydrologic Balance: <br />The pit excavation is located on a gravely terrace south of the Huerfano River. There was no evidence of excessive <br />erosion. There appeared to be no significant impacts to the prevailing hydrologic balance. <br />Gen. Compliance With Mine Plan: <br />The last annual report indicates the site has not been operational since August 2010. The Site has been inactive for <br />more that 180 days. This will be cited as a problem in this inspection report and will require corrective action. <br />Roads: <br />The access road was in good condition. <br />Recl Plan /Comp: <br />The operator has successfully backfilled and graded the majority of the current excavation's walls to a slope of 3H:1V or <br />flatter. <br />Revegetation: <br />There were annual weeds and a few thistles observed during the inspection. There was some plant growth on the <br />topsoil stockpiles and very little vegetation growth elsewhere in the disturbed area. The growth appeared to be <br />volunteer vegetation. <br />Erosion /Sedimentation: <br />There was no evidence of excessive erosion (rills, gullies, or sediment fans). <br />Signs and Markers: <br />A mine i.d. sign was posted at the entrance to the site. <br />Topsoil: <br />There was a topsoil stockpile along the western and eastern edges of the pit. <br />Page 2 of 4 <br />
