PERMIT #: P- 2009 -004 <br />INSPECTOR'S INITIALS: MAC <br />INSPECTION DATE: August 7, 2013 <br />OBSERVATIONS <br />The inspection was conducted by Michael Cunningham of the Division of Reclamation, Mining and Safety <br />(Division). John Crowley was present on behalf of the Operator. The LGPD #2 is located in Park County, <br />approximately 1.2 miles north of Lake George in the Pike -San Isabel National Forest. A USFS representative <br />was not present during the inspection. <br />On July 16, 2013, the Division received a request for release from further reclamation responsibility and <br />release of financial warranty; the notice was sent in accordance with Rule 4.16.1. The Division provided the <br />USFS a copy of the release request on August 6, 2013 per the terms of the Memorandum of Understanding <br />between the two agencies. The Division and the USFS must concur on the determination of whether the site <br />has been reclaimed in accordance with the approved NOI application prior to releasing the site. <br />The prospecting activities consisted of excavating four trenches which were approximately 6' x 6' x 20' L. The <br />trenches were backfilled in 2009 to match the existing contour of the surrounding area. The stockpiled topsoil <br />was replaced and the Operator placed dead trees along the contour to help prevent erosion until the <br />vegetation was established. The disturbed area was seeded with the approved seed mix in 2009 and again in <br />2010. The vegetation is well established and is at least equal in extent to the cover of the surrounding area. <br />The vegetation is capable of self regeneration and will continue to fill in with quickly as a result of the above <br />average rain fall in the spring and summer of this year. <br />Based on the observations made during the inspection, the Division has determined that the Operator has <br />successfully completed reclamation at the site. Notice of the Division's decision regarding the release request <br />will be sent under separate cover. <br />This concluded the inspection. <br />Page 2 of 3 <br />
