The first extension was addressed <br />by the Division's letter dated February 4, 2013 extending closure through September 2014. This recent request <br />(second request) was made through the following correspondence presented to the Division by the Facility. <br />Extension Request for Closure of Climax Mine Adit Vent Raise Asbestos Disposal Shaft. Prepared <br />by: Climax Molybdenum Company. Document dated: July 29, 2013. Document received: July 29, <br />2013. <br />The Division has reviewed this second extension request which would reschedule the closure plan activities to be <br />completed by September 2016. Based on this review it is the determination of the Division that a request be made <br />of the Facility to complete this activity no later than September 1, 2015 with the conditions that the following <br />conditions: <br />1) All structures and protective measures are fully maintained to ensure protection of human health and the <br />environment. <br />2) The adit shaft cannot be used for any disposal of any kind of additional materials. <br />3) The approved closure Plan will be followed as approved by the Division's conditional approval letter <br />dated January 8, 2013 during final closure activities. <br />
