On February 8, 2013, the Division of Reclamation, Mining and Safety received an Application Form for Transfer of Mineral Permit and
Succession of Operators for North River Gravel Pit Extension, Permit M- 1999 -007 and Noel's Pit, Permit M- 2008 -032. On February
12, 2013 you were notified that only a change in officer is required since Ace West Trucking, Inc. will remain the operator and asked to
provide:
Executed Affidavit of Authority for both permits
Copy of corporate meeting minutes designating new officers for Ace West Trucking, Inc.
As of this date, The Division has not received your documents to complete your change of officers. A second request was mailed to you
on June 12, 2013 again with no response.
You must provide the required documentation to complete your change request. Donald Rooks will remain the corporate officer of
Ace West Trucking, Inc. until the above documentation is received. Additional required documents includes:
1. Performance Warranty for each permit site
2. Financial Warranty in the amount of $25,900.00 for M -1999 -007 North River Gravel Pit
3. Financial Warranty in the amount of $33,631.00 for M -2008 -032 Noel's Pit
