PERMIT #: M- 1986 -159 <br />INSPECTOR'S INITIALS: TC1 <br />INSPECTION DATE: July 25, 2013 <br />OBSERVATIONS <br />This inspection was initiated by a request for Temporary Cessation. The purpose of this inspection was to check <br />compliance of the operation with the requirements of the Act, Rules, and the conditions of the permit. Rusty <br />Cochran was present during the inspection representing the Operator. This site is located approximately 6 '/2 <br />miles southeast of Pueblo, in Pueblo County. The site is accessed from Cortner Road. <br />Inspection: <br />A mine identification sign was posted (Photo 1), but needs the MLRB Permit No. Mr. Cochran brought a <br />Permit ID sign to attach, but it was for a different pit. The Division noted the entrance sign did not include a <br />permit number during the previous inspection on April 10, 2012. As this has not been remedied, it is cited as a <br />problem on page one of this report. <br />There has been no activity on site since the previous inspection. <br />Some standing water was observed in the Phase II and Phase III pits (Photos 2 and 3). This water can be <br />attributed to a 2 -inch rainstorm the night before the inspection. The storm was noted by the operator of the Big <br />G Pit during a subsequent inspection on July 25th, 2013. <br />Several tamarisk were observed in both the Phase II and Phase III pit areas. This is cited as a problem on page <br />one of this report. <br />The severe erosion observed during the previous inspection in the drainage channels adjacent to the haul roads <br />has been fixed with grading and the installation of large check dams (Photos 4 — 5). <br />Current Stipulations: <br />None. <br />Records: <br />• The previous inspection was conducted 4/10/2012. A problem was cited for severe erosion and resolved <br />on 1/30/13. <br />• The permit anniversary date is June 15. The operator is current with annual fees and reports through <br />June 2013. <br />Bond: <br />The current $73,556 bond is considered adequate to cover the cost of reclamation. <br />Summary and Recommendations: <br />1. The OPERATOR shall remove the tamarisk in both the phase II and phase III areas by the corrective <br />action due date on page 1. <br />2. The OPERATOR shall add "MLRB Permit No. M- 1986 -159" to the entrance sign by the corrective <br />action due date on page 1. <br />3. The DIVISION will recommend to the Board that the site be approved for temporary cessation. <br />Page 2 of 4 <br />
