PERMIT #: M- 2000 -039 <br />INSPECTOR'S INITIALS: TOD <br />INSPECTION DATE: July 19, 2013 <br />OBSERVATIONS <br />This was a follow -up inspection to determine if the current disturbance is within the permit boundary. Mr. <br />James Lott was contacted by Tim Cazier of the Division to notify Del Hur Industries of our intent to collect <br />GPS data on the recent survey stakes. Mr. Lott indicated he would not be present for the inspection. Tyler <br />O'Donnell represented the Division for the inspection. <br />This site is located approximately 3.5 miles southeast of the intersection of Colorado Hwy 10 and County Road <br />(CR) 210, which is about eight miles northeast of Walsenburg, in Huerfano County. The mine entrance road is <br />approximately 1.5 miles southeast of the intersection of CR 210 and CR 211. This is 110c limestone and shale <br />mining operation. <br />Inspection: <br />Survey Stakes — Field Data Collection: <br />Mr. O'Donnell used a Trimble Juno GPS receiver for field data collection. Survey stakes for points 1 — 7, and <br />10 were located and recorded with the GPS (see Figure 1). Points 8 and 9 could not be located. <br />Summary and Recommendations: <br />1. The attached Figure 1 demonstrates to the satisfaction of the DIVISION that the current mine <br />disturbance is within the permit boundary. <br />2. The OPERATOR shall permanent boundary markers prior to the resumption of mining. <br />3.
