PERMIT #: M- 2005 -044 <br />INSPECTOR'S INITIALS: MAC <br />INSPECTION DATE: July 15, 2013 <br />OBSERVATIONS <br />The inspection was conducted by Michael Cunningham of the Division of Reclamation, Mining and Safety <br />(Division). John DeWitt, the Lincoln County Land Use Administrator, was also present for the inspection. The <br />Hoffman Pit is located 1 mile east of Genoa, Colorado. The site is permitted for 9.9 acres and the post- mining <br />land use is rangeland. <br />On July 11, 2013, the Division received notification from Lincoln County that areas outside of the Hoffman Pit <br />permit boundary may have been affected by the mining operation. The purpose of the inspection was to <br />determine if any areas outside of the permit boundary had been affected by the mining operation. <br />The permit sign was posted at the site entrance and the boundary of the affected area was clearly delineated <br />as required by Rule 3.1.12. During the inspection, the Division traversed the affected area with a Trimble Juno <br />GPS unit. In addition, the locations of the corner makers for the permit boundary were also surveyed with the <br />GPS unit. The data were downloaded using GPS Pathfinder Office and the traversed areas were overlaid onto <br />an aerial photograph of the site; a copy of the map has been enclosed with this report. The Division <br />determined that 0.3 acres of land had been affected outside of the permit boundary. The enclosed map shows <br />two separate affected areas which are outside of the permit boundary. <br />The Operator has indicated that a Conversion Application will be submitted to increase the size of the pit <br />beyond the current 9.9 acre permit boundary. Due to the relatively small area that was affected outside of the <br />permit boundary and the Operator's intent to submit a Conversion Application, this matter has been cited as a <br />problem. Please see the first page of this report for additional information regarding the corrective action. The <br />Operator should note that if the corrective action is not complied with by the date specified, then this <br />problem could be escalated to a possible violation. <br />While traversing the perimeter of the pit, the Division observed rills and gullies on western edge of the permit <br />boundary along the west side of the haul road. Stormwater runoff at the site flows south and to the west into <br />a natural drainage channel which is located approximately 20 ft. west of the haul road. Stormwater from a <br />recent precipitation event was ponded within the pit. There is currently a berm located on the west side of the <br />haul road; however, the berm is not of sufficient height or length to adequately prevent stormwater runoff <br />from leaving the affected area. This same issue was cited as a problem during the Division's last inspection on <br />March 16, 2011. This matter has been cited as a problem. Please see the first page of this report for additional <br />information regarding the corrective action. Failure to adequately control stormwater runoff at the site in the <br />future may result in a possible violation. <br />This concluded the inspection. <br />Page 2 of 4 <br />
