PERMIT #: M- 2000 -018 <br />INSPECTOR'S INITIALS: TOD <br />INSPECTION DATE: June 26, 2013 <br />OBSERVATIONS <br />This was a normal routine monitoring inspection conducted by Tyler O'Donnell of the Division of Reclamation, <br />Mining and Safety (Division). Jerry Hoss, the mine operator's representative was present during the <br />inspection. The Munsch Pit is located approximately 4 miles southwest of Cheyenne Wells Colorado. The <br />Munsch Pit is permitted as a sand and gravel pit. The approved permit area is 9.5 acres. The approved post - <br />mining land use is rangeland. <br />At the time of the inspection it was sunny. The ground was dry. The current disturbance appeared to be <br />roughly 8 acres. The last annual report indicates the site has not been operational since October 2010. No <br />problems or violations were noted during the inspection. <br />Backfilling and Grading: <br />The majority of the site has been backfilled and graded with slopes of 3H:1V or flatter. All slopes appeared to <br />be structurally stable. <br />Hydrologic Balance: <br />The bottom of the pit was dry. There was no evidence of excessive erosion. There appeared to be no <br />significant impacts to the prevailing hydrologic balance. <br />Gen. Compliance With Mine Plan: <br />The operation appeared to be following the approved mine plan. All mining activity appeared to be within the <br />permit boundary approved in amendment AM -01. <br />Roads: <br />The access road was in good condition. <br />Recl Plan /Comp: <br />This site has not yet been reclaimed. <br />Revegetation: <br />The vegetation at the site is primarily annual weeds. There was some plant growth on the topsoil stockpiles <br />and very little vegetation growth elsewhere in the disturbed area. The growth appeared to be volunteer <br />vegetation. <br />Erosion /Sedimentation: <br />There was no evidence of excessive erosion (rills, gullies, or sediment fans). <br />Signs and Markers: <br />A mine I.D. sign was located at the entrance to the mine site. The permit boundaries were not clearly marked, <br />the inspector did not observe any boundary markers around the pit. This will be cited as a problem in this <br />inspection report and will require corrective action. <br />Topsoil: <br />Topsoil was stockpiled along the northern and southern portions of the permit boundary. <br />Page 2 of 4 <br />
