Letter to Nick Michael 5 June 24, 2013 <br />AM -01 Application Adequacy Review File No. M- 1990 -057 <br />28. As required by Rule 6.2.1(2), all maps must be signed by a registered land surveyor, <br />professional engineer, or other qualified person. In addition, all maps must give the date <br />prepared. Please revise the maps to include a date and signature. <br />29. Figure 6 -2 does not appear to be printed at the listed scale (1" = 100'). Please print the map at <br />the appropriate scale or revise the scale. <br />30. The Reclamation Plan Map (Figure 6 -3) depicts the various stockpiles, roads and the mill <br />building. The Reclamation Plan Map must match the Reclamation Plan and depict the site <br />following reclamation; any of the above features which will not be present after reclamation <br />must be removed from the map. Please revise the Reclamation Plan Map accordingly. <br />31. As required by Rule 6.3.5(3), the Reclamation Plan Map must state the average thickness of <br />replaced topsoil. Please revise the Reclamation Plan Map accordingly. <br />32. The Reclamation Plan Map shows the final gradient of all reclaimed slopes. Please confirm <br />that the site will be graded to the proposed gradient. <br />6.3.6 Exhibit F — List of Other Permits and Licenses Required <br />33. The Lake County Conditional Use Permit requires the Applicant to provide I I parking <br />spaces. Please indicate if these will be paved parking spaces and indicate the location of the <br />parking spaces on Figure 6 -2. <br />34. Please indicate if the Lake County Conditional Use Permit will need to be reviewed and <br />revised if the 110d Hard Rock Reclamation Permit Amendment Application is approved and <br />the facility becomes a custom mill. <br />35. The Division has reviewed the Weed Control Plan which was formulated for Lake County. <br />This plan may be implemented for the purpose of complying with Rule 3.1.10(6). However, <br />the plan must be revised to state that methods of weed control will be employed for all <br />noxious weeds, not just those species on List A. In addition, provide a commitment to <br />implement the Weed Control Plan for the duration of the operation. <br />6.3.7 Exhibit G — Source of Legal Right to Enter <br />No comment. <br />6.3.8 Exhibit H — Municipalities Within a Two Mile Radius <br />No comment. <br />6.3.9 Exhibit I — Proof of Filing with County Clerk <br />No comment. <br />
