The Applicant, <br />Providence Mining, LLC, has had no contact with Teller County regarding this matter nor have <br />they provided a copy of the application to the Teller County Clerk and Recorder for review (as <br />stated in your notification letter). Accordingly, we cannot provide any comment about an <br />application that is not available for review in Teller County and about which the Teller County <br />government has no knowledge or information. <br />2. Further, please be advised that the requirement on page 7 (Certification) of the Designated <br />Mining Limited Impact (110d) Operation Reclamation Permit Application Form has not been met <br />as the Applicant, Providence Mining, LLC, has not applied for any approvals from Teller <br />County concerning this matter and therefore the application form to the Division contains a <br />misrepresentation of material fact concerning item 1 on page 7, which states the following: "All <br />necessary approvals from the local government have been applied for (Rule 1.6.2(1) and (2)). <br />If you have any questions, please contact me. <br />Yeryruly yours, <br />yMianice E.
