PERMIT #: M- 2009 -069 <br />INSPECTOR'S INITIALS: BMK <br />INSPECTION DATE: June 6, 2013 <br />OBSERVATIONS <br />1) The Division conducted a monitoring inspection of the site on 6/6/13. The site is a 40 acre 112 sand and gravel <br />operation located 16 miles East of Holyoke Colorado. The site is operated by Yuma County. Present during the <br />inspection was Mr. Howard McGinnis of Yuma County Road & Bridge. <br />2) Permit sign and boundary markers were properly posted. <br />3) Approximately 4 acres were affected at the time of the inspection. All the drainage from the affected area is <br />captured with in the pit. Due to the high content of clay, sometimes the operator pumps the water from the pit <br />to meet the 72 hours maximum detention time allowed under the State Engineer's regulation. <br />4) The entire permit area was driven and inspected and found to be in compliance with the approved reclamation <br />plan. <br />5) Top soil stockpile was properly protected and no noxious weed infestation was observed. <br />Page 2 of 4 <br />
