PERMIT #: M- 1979 -189 <br />INSPECTOR'S INITIALS: BMK <br />INSPECTION DATE: April 26, 2013 <br />OBSERVATIONS <br />1) The Division conducted a monitoring inspection of the site on 4/26/ 13. The main purpose of the <br />inspection was to verify the additional reclamation work that will be required by the current operator, <br />before the land is transferred. The land is in the process of being purchase by the Trust for Public Lands <br />as a nature preservation area. For the city of Aurora. <br />2) Since the site is a permitted 112 sand and gravel operation that has not been active for over 10 years <br />the Division wanted to make sure all the additional reclamation work is identified and properly <br />documented prior to the transfer of the permit. <br />3) Present during the inspection representing the Division were Tony Waldron and Berhan Keffelew. <br />Representing the operator were Mike Hart and Bruce Peterson. Representing the Trust for Public Land <br />was Hillary Merritt. <br />4) The site for the most part appeared stable. The areas covered by the lake appeared to be supporting <br />migratory foul as observed during the inspection. <br />5) The Division identified additional reclamation work that will be needed to be conducted by the current <br />operator. They are as follows : <br />a) Remove all mining related equipment and associated items from the site. <br />b) Cut some of the bank slopes to approximate 3:1 slope <br />c) Make sure the exposed ground water that created the lake area does not exceed the approved 11 <br />acres. Provide a surveyed total exposed area to verify. The operator provided to the Division the <br />approved water augmentation plan via a mail on 4/27/13. <br />d) Repair the breached area along the east side. The Division is currently holding a warranty in the <br />amount of $114,850.00. It is deemed adequate to cover the current reclamation requirements. <br />Once these items are completed the operator can request an inspection to verify. Once the <br />additional reclamation work is completed, the SO (Succession of Operation) application can be <br />submitted. <br />Page 2 of 4 <br />
