PERMIT #: M- 2004 -031 <br />INSPECTOR'S INITIALS: BMK <br />INSPECTION DATE: May 30, 2013 <br />OBSERVATIONS <br />1) The Division conducted a monitoring inspection of the site to verify the location of the Agridrain system the <br />operator plans to implement to remediate the flooding issue in the adjacent property. On May 1, 2013, <br />Aggregate Industries submitted a technical revision # 2 replacing technical revision # 1 that was approved on <br />January 16, 2013. The purpose of the technical revision is to address the flooding issue in the adjacent land <br />owner's property caused by the installation of a slurry wall along the north side of the property by Aggregate <br />Industries. <br />2) Present during the inspection were, Tom Kaldenbach, Tony Waldron and Berhan Keffelew representing the <br />Division. Mike Refer, Connie Davis and Nick Fiberglas representing Aggregate Industries. <br />3) The operator's representatives explained the planned implantation of the Agridarin and how it is going to lower <br />the surface water from the adjacent land. Both the Division and the operator agreed that this was going to be a <br />permanent plan and when mining is completed the site will be transferred to the city of Thornton, with the <br />Agridarin system in place in the slurry wall along the north side. <br />4) The Division also observed the adjacent land that is still under water. During the inspection the operator agreed <br />to provide a detailed design of the structure and location and also request a 15 day technical revision decision <br />date extension to submit the design. In the afternoon of 5/30/13, the Division received a 15 day extension via e <br />mail and the decision date is changed from 5/31/13 to 6/14/13. <br />Page 2 of 4 <br />
