The following attachments are <br />included with this cover letter. <br />1. Annual fee check for $791.00, RECEIVED <br />2. Completed annual report form, and <br />3. Annual report map. MAY 3 �D13 <br />DMSION OF RECLAMATION <br />Mining and reclamation highlights from the report year include: MINING AND SAFETY <br />➢ On September 24, 2012 AM -01 was approved. The amendment added 197.51 acres to the <br />previous 89.16 acre 25th Avenue Site creating a total permit size of 286.67 acres. All of the <br />added acreage was part of an existing mining area known as the Greeley West Pit, permit <br />number M- 1977 -081. The added acreage has been previously disturbed by mining activities <br />and partially reclaimed. <br />➢ The permit size was reduced to 284.64 acres following the release of 2.03 acres from the <br />original 25`h Avenue Site area. The acreage is a portion of 35th Avenue right -of -way that had <br />been included in the original permit boundary. The acreage reduction was approved on <br />September 24, 2013. <br />➢ The two existing SW Ponds have been dewatered and are actively being mined as one <br />mining cell approximately 30 acres in size. Existing side slopes have been graded to 3H: IV <br />or flatter and 3H: IV side slopes are being maintained as mining progresses. <br />➢ Portions of the SE Pond have been backfilled to create two berms. The berms separate areas <br />of the pond to be backfilled from areas to be left as open water for final reclamation. The <br />pond was dewatered to perform the berm construction, but the pond is not currently being <br />dewatered. While the pond was dewatered, selected pond side slopes were graded to 3H: IV <br />for final reclamation. Areas above the anticipated pond high water level were seeded with <br />native grasses for weed control and final reclamation. <br />➢ There are no mining activities on the original 25th Avenue Site permit area <br />(approximately 87 acres).
