Mr. Craig Lis <br />GSL Sand and Gravel SWSP <br />March 28, 2012 <br />Page 3 of 5 <br />SWSP renewal request, the Applicant shall demonstrate continued diligence in this effort <br />including the status in obtaining the proper bonds through DRMS. <br />Conditions of Approval <br />I hereby approve this substitute water supply plan, in accordance with Section 37 -90- <br />137(11), C.R.S., subject to the following conditions: <br />1. This SWSP shall be valid for the period of November 1, 2012 through October 31, 2013, <br />unless otherwise revoked, modified, or superseded by decree. A SWSP renewal request <br />must be submitted to this office with the statutory fee (currently $257) by August <br />31, 2013. <br />2. A well permit was obtained for the current use and exposed surface area at the site in <br />accordance with §37 -90- 137(2) and (11), C.R.S., permit no. 57723 -F. <br />3. The total surface area of the groundwater exposed at the site must not exceed 26.7 acres <br />during the approval period of this SWSP, resulting in evaporative losses of 76.38 acre -feet <br />for this plan period. <br />4. This SWSP does not permit any water use for dust control, aggregate production or any <br />other purpose, other than evaporation. <br />5. Total consumption at the site must not exceed the aforementioned amounts unless an <br />amendment is made to this plan. <br />6. Approval of this plan is for the purposes as stated herein. Any additional uses of this <br />water must first be approved by this office. <br />7. All releases of replacement water must be sufficient to cover all out -of- priority depletions <br />in time, place, and amount and must be made under the direction and /or the approval of <br />the water commissioner. The release of replacement water may be aggregated to <br />maximize beneficial use. The water commissioner and /or the division engineer shall <br />determine the rate and timing of an aggregated release. <br />8. All diversions shall be measured in a manner acceptable to the Division Engineer. The <br />Applicant shall install and maintain such measuring devices as required by the Division <br />Engineer for operation of this SWSP. In addition, the applicant shall maintain daily <br />records of all diversions, replacements, and the amount of water used for each particular <br />purpose. The applicant shall provide a report of these records to the division engineer <br />(DivlAccounting(c�state.co.us) and the water commissioner (Bill Schneider at <br />William. SchneiderCaD-state. co. us) on a monthly basis. The accounting must be submitted <br />within 30 calendar days of the end of the month for which the accounting is being made <br />and conform to the Administration Protocol "Augmentation Plan Accounting, Division <br />One — South Platte River' (attached). <br />In addition, the applicant shall verify that the entities making replacements are identifying <br />this use on their accounting submitted to our office. For the period of this plan, that entity <br />is the City of Aurora (WDID 0802593 — Aurora Reuse). <br />9. The name, address, and phone number of a contact person who will be responsible for <br />the operation and accounting of this plan must be provided on the accounting forms to <br />the Division Engineer and Water Commissioner. <br />10. Conveyance loss for delivery of augmentation water to the South Platte River is subject <br />to assessment and modification as determined by the division engineer. <br />
