According to our <br />records, the following problems were to be resolved on or before April 21, 2013. Listed below are descriptions <br />and corrective actions for the problems. Please resolve the corrective actions and provide evidence of such <br />resolution on or before May 31, 2013. <br />Please note that if the corrective action is not properly addressed, and evidence provided to the Division by the <br />date provided above, the listed problems could be escalated to the level of possible violations and would then <br />be referred to the Mined Land Reclamation Board for further action. <br />PROBLEM /POSSIBLE VIOLATION: <br />The mine identification sign and affected area boundary markers were not observed per the requirements <br />of Rule 3.1.12. <br />CORRECTIVE ACTIONS: <br />The Operator must post a sign at the entrance to the mine site which shall be clearly visible from the access <br />road and specify the following; the name of the operator, indicate that a reclamation permit for the <br />operation has been issued by the Colorado Mined Land Reclamation Board, and the permit number. The <br />boundaries of the affected area must be marked by monuments or other markers that are clearly visible <br />and adequate to delineate such boundaries. Photographic evidence of the sign and markers must be <br />submitted to the Division by the corrective action due date. <br />If the problem has already been remedied, please provide evidence of its completion.
