In particular, the geotechnical observations and measurements will include evaluation of <br />the presence or absence of clay filling in geologic structures that dip out of the quarry face, and will evaluate any changes in <br />orientation, spacing, and roughness of geologic structures observed relative to the observations documented in the initial <br />report. <br />2. The Applicant must commit to preparing a blasting report for each shot. These reports must be retained by the Operator for <br />at least three years and be available for inspection by the DRMS on demand. The reports shall contain the following data: <br />a) Location, date, and time of blast; <br />b) Name, signature, and license number of blaster -in- charge; <br />c) Identification, direction and distance, in feet, from the nearest blast hole to the nearest potentially affected structure <br />either: <br />i. not located in the permit area; or <br />ii. not owned nor leased by the person who conducts the mining operations; <br />d) Type of material blasted; <br />e) Sketches of the blast pattern including number of holes, burden, spacing, and delay pattern. Sketches shall also <br />show decking, if holes are decked to achieve different delay times within a hole; <br />f Diameter and depth of holes; <br />g) Types of explosives used; <br />h) Total weight of explosives used per hole and maximum weight of explosives used per 8- millisecond period; <br />i) Initiation system; <br />j) Type and length of stemming; <br />k) Mats or other protections used; <br />I) Type of delay detonator and delay periods used; <br />m) Number of persons in the blasting crew; and <br />n) Seismographic records, where required, including: <br />i. Type of instrument, sensitivity, and the calibration signal of the gain setting or certification of annual <br />calibration; <br />ii. Exact location of instrument and the date, time and its distance fs-om the blast; <br />iii. Name of the person and firm taking the reading; <br />iv. Name of the person and firm analyzing the seismographic record; <br />v. The vibration level recorded. <br />A copy of the original approval letter is enclosed as Attachment B. <br />PERMIT #: M- 2007 -014 <br />INSPECTOR'S INITIALS: TC1 <br />INSPECTION DATE: March 14, 2013 <br />Recommendations: <br />1. The Division recommends the financial warranty liability be $113,900. The Operator should submit in <br />writing, a request for a Surety Reduction to the Division under separate cover. The request must include <br />an estimate of the actual cost to reclaim the site based on what it would cost an independent contractor <br />to complete reclamation. The Operator should include a copy of Task 003 enclosed in Attachment A to <br />meet this requirement. There is no fee for a Surety Reduction. <br />2. The Operator shall include the following information on the permit sign: <br />a. the name of the Operator, <br />b. a statement that a reclamation permit for the operation has been issued by the Colorado Mined <br />Land Reclamation Board; and. <br />c. the permit number. <br />3. The Operator should submit a Technical Revision (TR) to modify the reclamation seed mix to that <br />originally recommended by the NRCS (see table on Attachment C).
