Hickenlooper <br />Governor <br />Mike King <br />Executive Director <br />Dick Wolfe, P.E. <br />Director /State Engineer <br />RECEIVED <br />ADR 1 1 2013 <br />DIVISION OF RECLAMATION <br />MINING AND SAFETY <br />The Division of Water Resources (DWR) has reviewed the above - referenced Carbon Junction Mine <br />permit renewal application received by the Division of Reclamation, Mining and Safety (DRMS) on <br />February 1, 2013. The operator, Oakridge Energy, Inc., is requesting renewal of the mining and <br />reclamation permit. Carbon Junction Mine is an inactive mine located approximately 2 miles southeast <br />of Durango in La Plata County. <br />According to your March 27, 2013 correspondence, the permittee is requesting renewal of the permit as <br />they are currently awaiting successful establishment of revegetation and implementation of the post - <br />mining land use. Additionally, you provided information concerning five wells on the subject property: <br />Wells B17, B10, 94 -1, PW1, and 95 -1. According to this offices records, Wells B17, 94 -1, and PW1 are <br />currently permitted as monitoring wells under valid well permit nos. 201565, 201566, and 202084, <br />respectively. <br />Well 95 -1 was acknowledged for construction under monitoring hole notice MH -25308 and later <br />repermitted as a monitoring well under permit no. 201564. It appears that this office never received <br />information regarding construction of the well under notice MH -25308 or permit no. 201564. In order <br />to validate permit no. 201564, the applicant shall submit to this office information regarding <br />construction of the well. Such information may be submitted by the applicant using the Existing Well <br />Information and Inspection Form, GWS -68, or by the contractor responsible for drilling the well using <br />the Well Construction and Test Report, GWS -31. I have enclosed copies of both forms for reference. <br />It appears that Well B10 does not have a valid well permit; therefore the well must be either permitted <br />or property plugged and abandoned in accordance with Rule 16 of the Water Well Construction Rules, 2 <br />CCR 402 -2. Application may be made to use the existing well for monitoring purposes by using the <br />Monitoring Well Permit Application, GWS -46, and including evidence of well construction, as outlined in <br />the previous paragraph. I have enclosed a copy of Form GWS -46 for reference. <br />Thank you for the opportunity to comment on this application.
