OBSERVATIONS <br />PERMIT #: M- 1973 - 007 -SG <br />INSPECTOR'S INITIALS: BMK <br />INSPECTION DATE: February 7, 2013 <br />1) The Division conducted a monitoring inspection of the site on 2/7/13. Present during the inspection <br />representing the operator were Mr. Mac Schafer and Mr. Andre La Roche. The site is a phased 112 sand and <br />gravel operation located along Academy Blvd. The total permitted acres is 352.8 acres. <br />2) Inspected the active settling pond that is used to deposit wash fines. It was accepting slurry at the time of the <br />inspections. <br />3) Inspected the active mining area along the south side known as "J Property ". No problems noted. All the three <br />major designated areas of the permit were inspected and found to be in good repair. Even though there are no <br />maximum height and slope provisions of the active mining faces, given the areas potential for slope failures, the <br />operator is encouraged to limit the high walls to avoid failures. <br />4) Inspected the reclaimed areas along the north side. They appeared stable and naturally vegetated. <br />5) On July 24, 2006 the Division approved acreage reduction # 1 which reduced the permit area by 94.35 acres to <br />343.15. The 94.35 acres were released because area is zoned Industrial Commercial by the city and county. No <br />reclamation activity is need because post mining lad use in this area will remain as industrial commercial. <br />6) The permit sign and boundary markers were properly displayed. All the drainage from the area is captured <br />within the permit boundary. According to Andre, the site is in the process of obtaining a new storm water permit <br />from the Colorado Department of Health and Environment. <br />Page 2 of 4 <br />
