(a) The name and address of the permitee and permit number: (b) Location and number of acres disturbed during that year: There were no new acres of disturbance at the Hayden Gulch Loadout during 2012. (c) Location and number of acres backfilled and graded during that year: There were no new acres backfilled and graded at the Hayden Gulch Loadout during 2012. (d) Location and number of acres topsoiled during that year: There were no new acres topsoiled at the Hayden Gulch Loadout during 2012. (e) The species, location and number of acres of vegetation planted during that year, including any augmented seeding or cultural practices: There were no new acres seeded at the Hayden Gulch Loadout during 2012. Location, number of acres and date of planting, for all previously revegetated areas: The two water pipeline corridors, which represent approximately 1.4 acres, have been backfilled, graded and seeded with a permanent seed mixture. The work was completed by H -G Coal Company, the previous owner /operator /permitee, in conjunction with the facility construction (circa 1979/1980). Hayden Gulch Terminal, LLC assumes that Seed Mix No. 2 (improved pasture mix - subirrigated /irrigated) was used for the corridor section found in Section 10, T6N, R88W. The pipeline corridor found in Sections 14 and 15, T6N, R88W was most likely seeded with Seed Mix No. 1 (dryland pasture mix). However, Hayden Gulch Terminal, LLC has been unable to locate
