OBSERVATIONS <br />PERMIT #: M- 1987 -171 <br />INSPECTOR'S INITIALS: BMK <br />INSPECTION DATE: September 26, 2012 <br />1) The Division conducted a monitoring inspection of the site at the request of the operator. Mr. Michael Holmes <br />of Kiewit construction contacted the Division requesting a site visit to see if the current warranty can be <br />reduced. <br />2) Present during the inspection were Michael Holmes representing the operator, Sonia Stephens representing <br />Lafarge Inc, who mined a portion of the affected area along the north side under the Kiewit permit and Berhan <br />Keffelew representing the Division. <br />3) The inspection began along the north side of the permit area where Lafarge conducted a mining and <br />reclamation operation. Most of the affect area by Lafarge has been reclaimed with the exception of a few <br />processed stockpiles still in place. The reclaimed pit slopes and associated affected areas looked stable and <br />moderately vegetated. No major noxious weed infestation was observed at the time of the inspection. Both <br />representatives agreed additional seeding would be required so adequate vegetation can establish for final <br />release. <br />4) Inspected the areas affected by Kiewit construction. All the backfilling and grading has been completed. All the <br />erosion features noted during the last inspection have been repaired. With the exception of an old grizzly, still <br />on site all the temporary structures have been removed from site. <br />5) If the operator submits a surety Reduction request in accordance with Rule 4.17 of the Construction Materials <br />Rules and Regulations, the Division will reduce the financial warranty by $277,000.00. The request has to come <br />separately via certified mail and must include the address of the current owner of the property. <br />Page 2 of 4 <br />
