This inspection was initiated by an application for succession of operator (SO). The focus of this inspection <br />was to assess the operation status and determine what, if any activities might have an impact of the financial <br />warranty liability. Mr. John Sliman and Mr. Joe Gagliano represented the new Operator (Southwest Farms, <br />Inc.). <br />The Blue Grass Gravel Pit is located in Pueblo County, approximately 10 miles east of Pueblo, CO. The access <br />road is 1.8 miles east of the intersection of U.S. Hwy 50 and State Hwy 231. This permit consists of two non- <br />contiguous parcels referred to as Stage 1 (182 -acre east parcel) and Stage 2 (141 -acre west parcel). <br />Inspection: <br />A permit sign was prominently displayed at the east portion site boundary (see Photo 1). The Operator name <br />on the sign will need to be changed from Beltramo Leasing, LLC to Southwest Farms, Inc. The permit <br />boundary is marked with fences and T -posts (see Photo 2). The site is not currently active. Approximately <br />14.5 acres on the south end of Stage 1 were stripped for mining in 2010, but no mining occurred. This stripped <br />area has been backfilled level with the surrounding topography and the topsoil replaced, but has not been seeded <br />(see Photos 3 and 4). <br />No exposed groundwater was observed, nor was any stormwater retained onsite. No noxious weeds were <br />observed. <br />File Review: <br />• The previous inspection was performed on November 17, 2010. No problems were noted. <br />• The total permitted area is 323 acres of which 21.40 acres may be affected area. <br />• Annual fees and reports are up to date through 2012. <br />• The existing bond is a $48,600 based on the Technical Revision TR -01 request to reduce the affected <br />area to 21.4 acres and avoid exposing groundwater by limiting mining depth to no more than 10 feet. <br />• An updated (i.e., with 2012 unit costs) reclamation cost estimate ($48,030) for the site is enclosed. <br />Unit costs are updated from the original 2010 estimate for TR -01 and corrected earthworks volumes <br />(assuming 4 inches of overburden and 4 inches of topsoil, each over 21.4 acres as approved in TR -01). <br />Current Stipulations: <br />None. <br />OBSERVATIONS <br />PERMIT #: M- 2008 -001 <br />INSPECTOR'S INITIALS: TC1 <br />INSPECTION DATE: March 13, 2013 <br />Recommendations: <br />1. The Division recommends the financial warranty liability remain $48,600. <br />2. The Operator should replace the existing permit sign with one that includes the permittee's company <br />name. <br />3. Based on discussions during this inspection, the Operator should apply for Temporary Cessation prior to <br />this year's anniversary date (November 21) when the annual fee and report are due. <br />Page 2 of 3 <br />
