OBSERVATIONS <br />PERMIT #: M -1977 -342 <br />INSPECTOR'S INITIALS: PSH <br />INSPECTION DATE: February 26, 2013 <br />The Henderson Mill was inspected by Peter Hays with the Division of Reclamation, Mining and Safety (Division) <br />as a follow up inspection to the notice by Tim Haynes with Climax Molybdenum of a release of tailing slurry <br />from the Henderson Mill. Mr. Tim Haynes with Climax Molybdenum — Henderson Operations (Henderson) <br />was present during the inspection. <br />Henderson is required by Rule 8.1(b) to notify the Division of a failure of any Environmental Protection Facility <br />(EPF) designed to contain or control designated chemicals or process solutions as identified in the permit. The <br />Tailing Delivery Line (TDL) is indentified as an environmental protection facility within the Mill Process Water <br />System in Henderson's Environmental Protection Plan. Mr. Haynes informed the Division of the tailing slurry <br />discharge on February 21, 2013 at approximately 4:33 pm by phone. <br />Tailing slurry was released through dislodged joint packing between two joints of reinforced concrete pipe <br />(RCP) associated with the Tailing Delivery Line (TDL). The leak was discovered by Henderson staff at 7:30 am <br />on Thursday February 21, 2013. Henderson employees initially wedged more packing into the joint to slow <br />down the escaping flow to approximately 1 gpm. Henderson staff estimated approximately 1,000 to 2,000 <br />gallons of tailings were released on to the TDL road. The leaked flow was observed for 250 to 300 feet down <br />the road. The drainage in this area reports to the 3 -Dam bench. No evidence of roadway or berm erosion was <br />noted during the inspection; however site observations were limited due to snow cover. <br />An investigation by the Henderson engineering staff determined the leak came from the pipe packing which <br />became dislodged inside the joint between two pieces of reinforced concrete pipe. Contraction of the HDPE <br />piping down gradient of an anchor block directly below the joint in question may have had some influence. No <br />tailing slurry left the affected area boundary. <br />On Friday February 22, 2013, tailing discharge was diverted through the #1 Cutout to alleviate pressure on the <br />damaged joint while a concrete patch was poured over the section of piping and the adjacent anchor block. <br />The concrete was allowed to set before tailing flow was routed through the section of pipe. Henderson <br />reported no sign of flow from the joint or adjacent areas of piping on February 26, 2013. Henderson already <br />planned to replace the damaged RCP piping with HDPE piping from the existing transition to Drop Tower 17. <br />This change will remove Tower 16 and require Henderson to submit a technical revision to the Division. <br />During the inspection, the Division observed the repaired section of pipe, the anchor block and the connection <br />between the RCP pipe and HDPE pipe. The repair was covered with concrete blankets at the time of the <br />inspection. Climax committed to provide a follow -up report to the Division as required by Rule 8.2.3.
