Summary and Resolution of Interaction:
As required by Rule 3.1.13 — Spill Reporting
Mr. Tim Haynes notified the Division of a spill of tailing slurry on February 21, 2013. The leak
was discovered at approximately 7:30AM on Friday 21, 2013.

(d) Tailing slurry
(e) An estimated 1,000 to 2,000 gallons of tailing slurry was released due to a joint
failure in the Reinforced Concrete Pipe delivery line. The released tailings did not
leave the permit area (or affected area), the released tailings flowed down the adjacent
road and returned into the 3 -Dam facility.
(f) Henderson employees wedged more packing into the void between the RCP pipes to
slow the release of tailing slurry. Tailings delivery was diverted to the #1 Cutout the
next day while failure was repaired.
Additional information: The Division will conduct an inspection of the spill on Tuesday
February 26, 2013.
