August, IM Local Jurisdiction Delta ( QUntg <br />MTCE section/huol 30233 <br />DOH Permit Na 383071 <br />M]SRMIR FRS PAID IN FIILL DN to -06 -, Required PwMk Few 974 On <br />STATE DEPARTMENT OF HIGHWAYS <br />PERMIT FOR ACCESS <br />The property owner, (herein called permittee) is granted permission <br />to construct an access approach on the earth side of State Highway 133 , a distance of 300 <br />Feet west from milepost 13 also known as —11335 T.ann 43rs_ Rt3wia cD for the <br />of obtainin access to mat 'tin-ling purpose g �fwr� 1 i i . The access approach shall be constructed, <br />maintained and used in accordance with the State Highway Access Code, the terms and conditions of this permit, site <br />requirements noted below, and listed attachments. This permit may be revoked by the Issuing authority [tat any time the <br />permitted access approach and Its use violate any of the terms and conditions of this permit or the State Highway Access <br />Code, The use of advance warning and construction signs, flashers, barricades and flagmen are required at all times <br />during access approach construction within State right -of -way in conformance with the MANUAL ON UNIFORM <br />TRAFFIC CONTROL DEVICES, Part VI. The Department of Highways and its duly appointed agents and employees shall be <br />held harmless against any action or damage sustained by reason of the exercise of this permit. <br />SITE REQUIREMENTS <br />1. Contractor shall follow the applicable construction specifications set for by the Department <br />of Highways in the latest manual Standard Specifications for Road and Bridge Construction. <br />2. Any damage to any existing Highway facilities shall be repaired prior to continuing other <br />work. <br />3. Highway widening for acceleration /deceleration lanes will be time as per plans aubmitt <br />to the Department of Highways with the full 64 -foot wide section. emeerAdoy <br />j. ptaepsrig 'i-es-sith the following material placed for final grader 12 inches ABC, Class <br />It 6 inches ABC, Class 6; and 4 inches of Hot Bituminous Pavement, Grading E. <br />4. Compaction of stub- grade, embankments and backfill shall comply with Section 203.11 of the <br />Division of Highways Standard Specifications. <br />5. Compaction of Hot Bituminous Pavement (HBP) shall comply with Section 401.17 of the <br />Division of Highways Standard Specifications. <br />6. If frost is present in the sub- grade, no surfacing material shall be placed until all <br />frost is gone or removed. <br />7. No drainage from this site shall enter onto the surface of the Highway. <br />S. All existing drainage structures shall be extended to accommodate all new construction <br />and safety standards. <br />9. The first 20 feet beyond the closest highway lane, including speed change lanes, shall <br />11 slope down and away from the highway at a 2% grade to ensure proper drainage control. <br />10. The new pavement shall slope on the same plane an the present pavement surface. <br />11. Fill /cut slopes shall be at a 2:1 slope on the roadway and at 6:1 on the access approach. <br />12. Tha area around the new work shall be well graded to drain, top soiled, fertilized, <br />mulched and reseeded. <br />13. Permanent highway striping and signing shall be done by the Colorado Division of Highways. <br />The actual cost of the work will be billed to the permitter. It will not exceed an <br />estimated $500. <br />14. All construction shall be completed in an <br />expeditious and cafe manner and finish within 45 days from initiation.
