PERMIT #: M- 1987 -171 <br />INSPECTOR'S INITIALS: BMK <br />1N, 1 = ECTION DATE: September 26, 2012 <br />OBSERVATIONS <br />1) The Division conducted a monitoring inspection of the site at the request of t e operator. Mr. Michael Holmes <br />of Kiewit construction contacted the Division requesting a site visit to see if ti-- current warranty can be <br />reduced. <br />Sonia Stephens representing <br />2) Present during the inspection were Michael Holmes representing the operatc7 <br />Lafarge Inc, who mined a portion of the affected area along the north side u n cl er the Kiewit permit and Berhan <br />Keffelew representing the Division. <br />3) The inspection began along the north side of the permit area where Lafarge c4 nducted a mining and <br />reclamation operation. Most of the affect area by Lafarge has been reclaimed with the exception of a few <br />cted areas looked stable and <br />processed stockpiles still in place. The reclaimed pit slopes and associated aff� <br />moderately vegetated. No major noxious weed infestation was observed at th e time of the inspection. Both <br />representatives agreed additional seeding would be required so adequate ve getation can establish for final <br />release. <br />4) Inspected the areas affect by Kiewit construction. All the backfilling and gra has been completed. All the <br />erosion features noted during the last inspection have been repaired. With th e exception of an old grizzly, still <br />on site all the temporary structures have been removed from site. <br />of the Construction Materials <br />5) If the operator submits a surety Reduction request in accordance with Rule 4. ] 7 <br />Rules and Regulations, the Division will reduce the financial warranty by $277,o o0.00.
