Please confirm that Washington County will continue to utilize the existing site entrance. <br />6.4.2 Exhibit B — Index Map <br />No comment. <br />6.4.3 Exhibit C — Pre - Mining and Mining Plan Map(s) of Affected Lands <br />3. Please revise the Pre - mining and Mining Plan Map to portray the following information: <br />a) All adjoining surface owners of record. <br />b) The name and location of all creeks, roads, buildings, oil and gas wells and lines, power <br />communication lines, and railroad lines within two hundred (200) feet of all boundaries <br />of such area. <br />c) The existing topography of the area with contour lines of sufficient detail to portray the <br />direction and rate of slope of the affected land. <br />d) The total area to be involved in the operation, including the area to be mined and the area <br />of affected lands. <br />e) The type of present vegetation covering the affected lands. <br />f) Show the owner's name, type of structures, and location of all significant, valuable, and <br />permanent man -made structures contained on the area of affected land and within two <br />hundred (200) feet of the affected land. <br />g) Include the name of the Applicant. <br />h) Include the date prepared. <br />i) Include the signature of the person who prepared the map. <br />j) Include an appropriate map legend, north arrow, and map scale (no larger than 1 inch = <br />50 ft. nor smaller than 1 inch = 660 feet). <br />k) Include a labeled permit boundary for the entire site (existing permit area & proposed <br />amended area). <br />1) Indicate the direction that material extraction will proceed. <br />m) Outline and label existing disturbance within and /or adjacent to the permit boundary. <br />n) Outline and label any easements (railroad, overhead power lines) within the permitted <br />area or within 200 ft. of the affected area. <br />6.4.4 Exhibit D — Mining Plan <br />4. The mine operation is located within the channel of the Surveyor Creek, an ephemeral <br />stream. Please commit to not mining within Surveyor Creek when the creek is flowing. In <br />addition, please commit to not placing stockpiles or storing equipment within the channel of <br />Surveyor Creek. <br />5. Please specify if any product will be stockpiled onsite. If so, depict the product stockpiles on <br />the Mining Plan Map. <br />
