Since I was looking at the 2011 AHR, I did a complete review, and have the following <br />comments: <br />1. The report provided by OMI is limited to a compilation of data with no <br />accompanying narrative. In future reports, under Rule 4.05.13(4)(c), the <br />Division will require that OMI include a written interpretation of the data <br />and identification of mining related impacts to the hydrologic balance. <br />2. Water monitoring frequency requirements were met at all surface and <br />groundwater monitoring sites except well D2132. Exhibit 2.05 -E7 of the permit <br />application package, the Hydrologic Monitoring Plan, indicates that field <br />parameters (flow, Total Suspended Solids (TSS), and pH) and water levels will <br />be measured twice during the June through October period at Wells D213 and <br />D2132 if the sites are accessible. OMI provided no data for D2132 in 2011; it <br />appears that D2132 has not been monitored since November 11, 2008. OMI <br />needs to revise the 2011 AHR to provide the required monitoring frequency <br />data for D21132, indicate that no water was transferred /the well was dry, or <br />explain why this well was not monitored as required. <br />3. Field parameters were collected and laboratory analyses were conducted as <br />required at all groundwater and surface water sites with the following exceptions: <br />a. Exhibit 2.05 -E7 of the permit application package, the Hydrologic <br />Monitoring Plan, indicates that field parameters (flow, TSS, and pH) and <br />water levels will be measured twice during the June through October <br />period at D2b and D2132 if the sites are accessible and water is being <br />transferred. OMI provided only water level information — no field <br />parameters - for D132 (February, June, and October), and no data for <br />D2132. OMI needs to revise the 2011 AHR to provide water level data <br />for D21132, and the required field parameter data (if water transfers <br />occurred) for D2113 and D21132 or explain the absence of this data. <br />b. Table 2.04.7 -T1 of the permit application package is the list of required <br />monitoring parameters for surface and groundwater monitoring. The list <br />requires reporting of both alkalinity and bicarbonate alkalinity. None of the <br />2011 monitoring data includes numeric values for alkalinity (CaCO3)., <br />graphs of alkalinity are presented for some of the monitoring sites. OMI <br />needs to revise the 2011 AHR to include this required data. <br />C. The 3rd quarter data for NF -3 was missing iron (Fe), both total recoverable <br />and dissolved. OMI needs to revise the 2011 AHR to include this <br />required data. <br />d. Rule 4.05.13(4)(b) states that data collection shall be based upon a water <br />year (October 1 — September 30).
