January 23, 2013 C-1981-014/Southfield Mine JHB <br /> ENFORCEMENT ACTIONS/COMPLIANCE <br /> On August 8, 2012,the Division directed Energy Fuels to remove the blockage from monitoring well <br /> MW-NW. Energy Fuels documented that they removed a tangle of steel cable from MW-NW on August <br /> 27, 2012. The Division documented the condition of MW-NW on September 17,2012 using a borehole <br /> camera. The Division observed a block of wood in the well at 353 ft.below the top of the casing. The <br /> Division sent another letter to Energy Fuels on November 28, 2012 directing Energy Fuels to clear the <br /> obstruction from MW-NW prior to the end of 2012 (December 31, 2012), and provide documentation to <br /> the Division that this had been accomplished. On December 31,2012 Energy Fuels submitted an <br /> application for a technical revision requesting the elevation of MW-NW monitoring point to coincide with <br /> the elevation of the obstruction. Energy Fuels provided no evidence that the obstruction had been cleared <br /> from MW-NW. <br /> Observation with the borehole camera on January 23,2013, confirmed that an obstruction remains in <br /> water monitoring well MW-NW.
