Box 68 <br />Empire, CO 80438 <br />Phone (303) 569 -3221 <br />RECEIVer <br />TAN 16 2013 <br />Denver, CO 80203 <br />y - RECIAit <br />nomworoAAID sot Re: ✓ oIIow -up Notice for Mine Water Line Leak, Permit No. M- 1977 -342 <br />Henderson Mine is providing this written follow -up notice to a phone call made to you on January 3, 2013 <br />pursuant to Rule 8.1 and 8.2 of the DRMS Rules and Regulations. <br />This notice is regarding a flow of mine water originating from one of the two lines that convey water from <br />the Henderson Mine to the URAD industrial wastewater treatment plant, which occurred on December 3, <br />2013 at approximately 1:OOpm. The Mine -URAD water lines are part of the environmental protection <br />facility 1.1 - Mine Water System, in Henderson's Environmental Protection Plan. The leak was <br />discovered at 1:00 pm. Approximately 1,000 gallons of Mine water leaked out. <br />After discovering the issue, Henderson employees determined that the flow was from the <br />12 -inch water line and promptly shut off the valves from the surge tank and the pumps <br />that bring water from underground to the URAD water line. <br />Mine water flow did not adversely impact human health, property or the environment. All <br />of the Mine water that leaked from the URAD water line made its way along an access <br />road and then puddled up at the lower portion of the access road, where it froze before <br />entering a nearby stormwater settling basin and outfall location. The ice has been <br />collected and placed in a lined settling pond (1.2 Pond). <br />The contact person for this event is: <br />Miguel Hamarat <br />P.O. Box 68 Empire, CO 80438 <br />Phone: 303 - 569 -3221 ext. 1233 <br />No monitoring or analyses were performed for this situation since Mine water was not <br />released off -site and did not impact any natural water body. <br />The investigation following the incident determined that the leak came from a 2 -inch PVC <br />line that collects condensate from the 5 -Shaft fans and feeds into the URAD water lines <br />for conveyance to URAD for treatment.
