2012 Repair Item No. <br />Location <br />Work Description <br />(Date(s) Repaired) <br />1 <br />Repaired fence at the Moffat Pond. Fence was <br />(05/07/12 and 05/25/12) <br />Moffat Pond Fence <br />damaged near the southwest corner, posts were bent <br />and the wires were lying on the ground. <br />Repaired contour ditch blowout near the north end of <br />2 <br />Contour Ditches <br />the site, approximately a quarter mile from the <br />(05/07/12) <br />Blowouts <br />boundary. The second ditch from the top contained a <br />blowout that was causing a large rill, approximately <br />one foot deep and four feet wide. <br />Intersection of the <br />Surveyed and authored a Memo detailing the ditch <br />3 <br />North Road and the <br />south of the intersection of the North Road and the <br />(No Repair Required) <br />Lower Moffat Road <br />Lower Moffat Road thought to have a low spot that <br />Ditch <br />could potentially pond and flow over the road rather <br />than drain to a culvert. <br />Replaced the West Ridge Pond discharge culvert in <br />4 <br />West Ridge Pond <br />accordance with MR -55. The primary spillway was <br />(10/04/12 to 10/06/12) <br />Discharge Culvert <br />rusted and contained a sizable hole approximately 10 <br />feet from the downstream end. <br />Repaired gullies and a roadside ditch where the West <br />Ridge Road crosses the West Ridge drainage area and <br />West Ridge <br />to the west of the drainage. The gullies were <br />5 <br />(10/09/12) <br />Gullies/Roadside <br />armored /filled with rip rap and the ditch was diverted <br />to proper orientation. The eastern gully was nearly <br />Ditch <br />200 yards long and at least 4 foot deep in places. The <br />western gully was approximately 60 yards long and <br />averaged 4 feet deep. <br />6 <br />Moffat Down Drain <br />Repaired rills at Down Drain #4. Rills had occurred in <br />(10/24/12) <br />#4 <br />a heavily traveled corridor related to livestock grazing. <br />Repaired rills and gullies at Moffat Final Pit Ditch. <br />7 <br />Moffat Final Pit <br />Minor rilling had developed on the north side of the <br />(10/24/12) <br />Ditch <br />channel. Two gullies had developed, both <br />approximately 30 yards long and 3 feet deep. <br />8 <br />Moffat Pond <br />Replaced both ends of the Moffat Pond discharge <br />(11/29/12) <br />Discharge Culvert <br />culvert, including trash racks. The primary spillway <br />was rusted at each end of the pipe. <br />9 <br />Lower Moffat Road <br />Repaired gully east of Lower Moffat Road. The gully <br />(10/9/12) <br />Gully <br />was approximately 100 feet long and 2 feet deep. <br />2012 Maintenance List <br />The Fourth Quarter 2012 Inspection was performed on December 12, 2012. All structures were <br />functioning as designed. <br />There are currently no maintenance items at the Edna Mine. <br />
