COLORADO DEPARTMENT OF PUBLIC HEALTH ANu ENVIRONMENT <br /> WATER QUALITY CONTROL DIVISION - BIOSOLIDS MANAGEMENT PROGRAM <br /> Annual Biosolids Report Contents for the Application of Biosolids <br /> Please provide the following information in your Biosolids Annual Self-Monitoring Report: <br /> (A) Documentation demonstrating compliance with the appropriate biosolids vector attraction <br /> reduction criteria contained in Section 64.12C of the Biosolids Re-tlations when the <br /> appropriate criteria are those contained in Section 64.12.C(11) through 64.12.C(13). <br /> (B) The results of any biosolids analyses performed pursuant to the requirements of Sections <br /> 64.16A and C of the Biosolids Relations and recorded on the enclosed form. <br /> (C) The results of any soils analyses performed pursuant to the requirements of Sections 64.163 <br /> and C of the Biosolids Re<lations and recorded on the enclosed form. <br /> (D) The results of any additional monitoring performed as specified in the Notice of Authorization <br /> for the Use and Distribution of Biosolids. <br /> ,,E) The total amount and disposition of biosolids applied during the preceding twelve months. <br /> including the date biosolids are applied :o each agricultural and/or disturbed land site issued a <br /> Notice of Authorization for the Use and Distribution of Biosolids, amount of biosolids applied <br /> to each agricultural and/or disturbed land site issued a Notice of Authorization for the Use and <br /> Distribution of Biosolids, and the cumulative amount of those pollutants identified in section <br /> 64.12.A, Table 1, of the Bio olids Regulations applied to each agricultural and/or disturbed <br /> land site issued a Notice of Authorization for the Use and Distribution of Biosolids. <br /> (F) Certification statement(s) required per Section 64.17.0 of the Biosolids Regulations as <br /> appropriate for the use and/or distribution option(s) practiced. <br /> (G) Short narrative description of biosolids practice(s), i.e. disposal, treatment. <br /> Armual Biosolids Reports must be submitted to the Division so the; are received by February 19th of each <br /> year.
