Annual Biosolids Report Contents for the Application of Biosolids
Please provide the following information in your Biosolids Annual Self-Monitoring Report:
(A) Documentation demonstrating compliance with the appropriate biosolids vector attraction
reduction criteria contained in Section 64.12C of the Biosolids Re-tlations when the
appropriate criteria are those contained in Section 64.12.C(11) through 64.12.C(13).
(B) The results of any biosolids analyses performed pursuant to the requirements of Sections
64.16A and C of the Biosolids Relations and recorded on the enclosed form.
(C) The results of any soils analyses performed pursuant to the requirements of Sections 64.163
and C of the Biosolids Re<lations and recorded on the enclosed form.
(D) The results of any additional monitoring performed as specified in the Notice of Authorization
for the Use and Distribution of Biosolids.
,,E) The total amount and disposition of biosolids applied during the preceding twelve months.

(F) Certification statement(s) required per Section 64.17.0 of the Biosolids Regulations as
appropriate for the use and/or distribution option(s) practiced.
(G) Short narrative description of biosolids practice(s), i.e. disposal, treatment.
Armual Biosolids Reports must be submitted to the Division so the; are received by February 19th of each
year. The Annual Biosolids Report must include the results for all items identified above for the period from
January 1 through December 31 of the preceeding year.
