You must provide copies of your Annual Biosolids <br /> Report to both the EPA and the Water Quality Control Division. You need only submit the first eight pages of the EPA <br /> reporting forms (labeled GENERAL FACILITY INFORMATION, CONTRACT APPLIER(S)/HAULER(S), BIOSOLIDS <br /> TREATMENT PROVIDED, FINAL USE/DISPOSAL, LAND APPLICATION SITE INFORMATION, CUMULATIVE <br /> LOADING, MONITORING DATA, PATHOGEN DESTRUCTION/VECTOR ATTRACTION REDUCTION) and the <br /> appropriate certification statement(s) to the EPA. The Report submitted to the Division must also include the additional <br /> information identified on the enclosed list entitled "Annual Biosolids Report Contents". You may utilize the enclosed <br /> reporting forms provided for biosolids and soils data or you may provide the information on lab sheets or your own <br /> spreadsheet. Please copy and attach additional pages as necessary. <br /> Please review the enclosed forms carefully, they have been modified from last year. Please fill out the forms completely, <br /> however,you may disregard the spaces filled with X's. Please do not remit any fees with your Annual Biosolids Report, an <br /> invoice will be sent to you soon after your Annual Biosolids Report has been returned. If you have paid some or all of your <br /> 1997 fee please note the date(s) and amounts of any payments already made. <br /> If you are utilizing a contract hauler you need only complete six pages of the EPA reporting forms (labeled GENERAL <br /> FACILITY INFORMATION, CONTRACT APPLIER(S)/HAULER(S), BIOSOLIDS TREATMENT PROVIDED, FINAL <br /> USE/DISPOSAL, MONITORING DATA,PATHOGEN DESTRUCTION/VECTOR ATTRACTION REDUCTION). Also, <br /> please include the additional information identified on the enclosed list entitled "Annual Biosolids Report Contents for <br /> Contracted Services of Biosolids". <br /> If your facility utilizes multiple biosolids use options, i.e. both composting and land application via a contractor, we have <br /> attempted to supply Annual Biosolids Report Contents forms for both options. If there has been a change in your use <br /> practices, however, the appropriate forms may have been omitted.
