I
have taken care of all of the mining permit business since 1965. I considered myself in good health with
the exception of always feeling tired when in one day things went bad for me. I went to the doctor for
blood work and spent the next month in the hospital at Rose Medical Center in Denver. I had several
life threatening illnesses and an error in a simple procedure put me in the ICU for three weeks and a
slow recovery at home. It was 6 months before I was back on my feet and I've struggled to get caught
up ever since. When going through papers for the end of the year I came across the mail from the
mining permits and I had not taken care of it. I apologize and any grace you can extend on starting the
enforcement process will be greatly appreciated. If not, I understand. Thank you.
RECEIVED
JAN 037013
DIVISION OF RECLAMATION
MINING AND SAFETY
