Compliance With Mine Plan R - Support Facilities Not On-site <br /> NA -Other NA - Special Categories Of Mining <br /> Y -Processing Waste Y -Topsoil <br /> COMMENTS <br /> This was a complete inspection of the Munger Canyon Mine conducted by Mike Boulay of the DRMS Grand <br /> Junction Field Office on December 20,2012. There was no operator/representative from Munger Canyon Mine <br /> present during the inspection. Weather conditions were sunny and cold. The ground was entirely snow covered <br /> from the storm that occurred prior to the inspection. <br /> AVAILABILITY OF RECORDS—Rule 5.02.4(l): <br /> The availability of records was checked at the J.E. Stover&Associates office on December 27,2012. All <br /> required records were on file and available for review with exception of a current Insurance Certificate. Please see <br /> the DRMS Availability of Records checklist at the end of this inspection report. <br /> ROADS—Rule 4.03 <br /> Construction 4.03.1(3)/4.032(3) <br /> Drainage 4.03.1(4)/4.032(4) <br /> Surfacing and Maintenance4.03.1(5)and(6)/4.032(5)and(6) <br /> Reclamation 4.03.1(7)/4.032(7): <br /> The mine site access road off of Highway 139 was deemed unsafe to travel on due to snow packed and icy <br /> conditions. The access road had not been plowed. There was no activity at the mine site and it did not appear that <br /> any one has attempted to travel onto the access road since the recent snowstorm. No problems were noted with <br /> this inspection. <br /> OFFSITE SUPPORT FACILIITES -Rule 4.04,428: <br /> The reclaimed Loma Loadout site was inspected. The site was entirely snow packed. The sign with operator <br /> information,DRMS Permit Number and MSHA ID is in good condition and clearly visible at the site. <br /> Number of Partial Inspection this Fiscal Year: 4 <br /> Number of Complete Inspections this Fiscal Year: 2 <br /> Page 2 of 6 <br />
