Compliance With Mine Plan NA - Support Facilities Not On-site <br /> N -Other NA - Special Categories Of Mining <br /> NA -Processing Waste N -Topsoil <br /> COMMENTS <br /> A partial inspection was conducted by Rob Zuber of DRMS,Dan MacKinnon of OSM, and Gordon Brocker, <br /> representing Energy Fuels. Other people were invited but did not attend: Douglas Barnett,primary landowner; Joe <br /> Sessions, adjacent landowner at the loadout; and Jennifer Maiolo of the BLM. <br /> The inspection covered the loadout and the mine site and was a bond release inspection for SL-05. There was <br /> minor snow falling, and the ground contained approximately two inches of snow. <br /> HYDROLOGIC BALANCE -Rule 4.05❑ Drainage Control 4.05.1,4.05.2,4.05.3; Siltation Structures 4.05.5, <br /> 4.05.6; Discharge Structures 4.05.7,4.05.10; Diversions 4.05.4;Effluent Limits 4.05.2; Ground Water <br /> Monitoring 4.05.13; Surface Water Monitoring 4.05.13; Drainage—Acid and Toxic Materials 4.05.8; <br /> Impoundments 4.05.6,4.05.9; Stream Buffer Zones 4.05.18: <br /> At the north culvert at the mine site,no problems were observed. At the south culvert,the smaller pipe is <br /> silted in,nearly completely,but the larger pipe is functional. <br /> The Best Management Practices for sediment control from the reclaimed pond area are in good shape. <br /> The two alluvial monitoring wells at the loadout have yet to be removed(as of the time of the inspection on <br /> December 17,2012). This will need to be done in compliance with the PAP(page 780-88b)prior to a DRMS <br /> approval of the SL-05 bond release application and final release of the site. A follow-up inspection by DRMS will <br /> need to be conducted to confirm that these wells were removed. <br /> GENERAL MINE PLAN COMPLANCE: <br /> The site was found to be in general compliance with the permit. <br /> ROADS—Rule 4.0311 Construction 4.03.1(3)/4.032(3)❑ Drainage 4.03.1(4)/4.032(4)11 Surfacing and <br /> Number of Partial Inspection this Fiscal Year: 4 <br /> Number of Complete Inspections this Fiscal Year: 2 <br /> Page 2 of 7 <br />
