Appendix A <br />Additional Information Required for the Schwartzwalder Injection Well Request <br />Per 40 CFR Section 144.26(b)(2), EPA is requiring the following information: <br />1. Location of each well or project given by Township, Range, Section, and Quarter - <br />Section, or by latitude and longitude to the nearest second, according to the conventional <br />practice in the State; <br />2. Date of completion of each well; <br />3. Identification and depth of the formation(s) into which each well is injecting; <br />4. Total depth of each well; <br />5. Casing and cementing record, tubing size, and depth of packer; <br />6. Nature of the injected fluids; <br />7. Average and maximum injection pressure at the wellhead; <br />8. Average and maximum injection rate; and <br />9. Date of the last mechanical integrity test, if any. <br />Per 40 CFR Section 144.27(a), EPA is requiring the following information: <br />1. Please provide the current chemical constituent concentrations in the mine pool. <br />2. Please provide the anticipated chemical constituent concentrations in the injected brine. <br />3. Please provide calculations on the resulting mine pool chemical constituent <br />concentrations. These should provide several intervals over the period this process is <br />proposed to take place. <br />4. Please provide the relative difference for the concentrations versus the initial mine pool <br />chemical constituent concentrations for the chemical constituent concentrations above <br />(Item 3). <br />5. As a proposed backfill project, please characterize the solid portion of the injectate and <br />percentage relative to liquid volumes. <br />6. Please provide a detailed description of the aquifer that this project will take place in and <br />the nearest underground sources of drinking water (USDW). This description should <br />include water quality data (chemical constituents and TDS), aerial extent, depth and <br />thickness, and hydrogeologic characteristics. <br />7. Please describe any hydrologic connections to any USDWs, and if there are none, please <br />explain why. The USGS did extensive research on this site and an evaluation of their <br />findings should be included in this discussion. Please provide citations for any <br />information or data sources used to answer this and Item 6. <br />8. Please provide an overview of the project with proposed time lines and mile stones <br />including anticipated duration. <br />9. Please provide general information related to any proposed remediation associated with <br />this project and include it in the timeline discussed in Item 8. <br />10. Please provide the location of the nearest private and public drinking water supply wells. <br />
