6.4 Defect and Repair Observations <br />6.5 Destructive Testing <br />ameel <br />AMEC inspected geomembrane panels for damage that may have occurred during <br />manufacture, transport, deployment, or installation as discussed in Section 6.1. Any <br />damage noted (during or after deployment) by AMEC was reported. AMEC repeatedly <br />observed all panels and seams for defects until Ames covered the geomembrane with DCF. <br />AMEC documented defects by assigning a unique defect identification code to each defect, <br />and marking each defect code on the geomembrane. The summaries of defects and <br />repairs, including the approximate locations of each repair, are presented in Appendix J.4 <br />for the 2011 Phase 5 VLF OSA 3rd Portion primary geomembrane layer. <br />AMEC selected destructive test locations based on Lone Star's completion of approximately <br />176,884 lineal feet of welding for the primary geomembrane, various working conditions, <br />and on seaming observations (e.g., suspicion of a defective weld). Lone Star obtained and <br />tested 348 fusion seam destructive samples and 34 extrusion seam destructive samples of <br />the primary geomembrane for the 2011 Phase 5 VLF OSA 3 Portion. <br />Lone Star constructed approximately 171,090 lineal feet of fusion welded seams and 5,794 <br />lineal feet of extrusion welded seams for the primary geomembrane. The specified <br />frequency of destructive testing was one test per 500 lineal feet of weld. AMEC identified <br />and Lone Star obtained destructive samples from the primary .geomembrane panel seams <br />at a frequency of one destructive test sample per 458 lineal feet of fusion welded seams, <br />and one per 170 lineal feet of extrusion welded seams. The test frequencies exceeded <br />project requirements. <br />Destructive test frequencies and quantities include seaming of panels that are not within the <br />2011 Phase 5 VLF OSA 3 Portion Ceritification Boundary.
