4.0 EARTHWORK CQA TESTING AND MONITORING <br />The following sections summarize results of the earthwork CQA testing and monitoring <br />performed by AMEC for the 2011 Phase 5 VLF OSA 3 Portion . <br />AMEC conducted CQA testing of earthwork materials used for construction. AMEC <br />performed CQA testing of in -place materials used for construction of the 2011 Phase 5 VLF <br />OSA 3 Portion. Table 4 presents specified and observed CQA testing frequencies. <br />Figure 1 shows the site plan and coordinate system used to locate tests. <br />In accordance with the AMEC Earthworks CQA Plan (Section 1400.1 of the Specifications), <br />when an AMEC monitor observed deficiencies, the monitor determined the nature and <br />extent of the problem and notified Ames and /or CC &V. When unsatisfactory test results <br />identified a deficiency, AMEC performed additional tests to define the extent of the deficient <br />area. Ames corrected deficiencies to meet the requirements of the specifications, and <br />AMEC retested and /or approved the corrected deficiency prior to any additional related <br />work. The following sections include discussions of the deficiencies encountered. <br />4.1 Results of Laboratory Soils Testing <br />AMEC performed laboratory testing of soils during all earthworks aspects of the 2011 Phase <br />5 VLF OSA 3 Portion construction. AMEC's on -site soils laboratory at CC &V performed <br />the material testing.
