Pineda <br />Director <br />On November 21, 2012, the Division of Reclamation, Mining and Safety (Division) received <br />your 112 Construction Materials Reclamation Permit Amendment Application package for the <br />Elrick Pit, File No. M -1997 -015. Preliminary review of the application package determined that <br />the following items must be received before the Division can consider your application as being <br />submitted and technical review can begin: <br />Application Form <br />1. The Applicant did not provide a general description (the number of miles and direction from <br />the nearest town and the approximate elevation) on Page 2 of the Application Form. Please <br />submit a replacement page and include a general description for the site. <br />2. The Applicant did not initial the `Responsibilities as a Permittee' section on Page 5 and Page <br />6 of the Application Form. Please provide a completed Page 5 and Page 6 of the Application <br />Form. <br />3. Page 8 of the Application Form did not include a date for the signature of the Applicant. <br />Please provide the Division with a revised copy of Page 8 which includes the date for the <br />signature of the Applicant. <br />4. The Applicant submitted copies of the original notices from 1997 which were provided to the <br />Board of County Commissioners and the Soil Conservation District.
