OBSERVATIONS <br />PERMIT #: M -1978 -011 <br />INSPECTOR'S INITIALS: BMK <br />INSPECTION DATE: October 29, 2012 <br />1) The Division conducted a monitoring inspection of the site on 10/29/12. The site is an old clay mine <br />that is located approximately 10 miles NE of Kiowa Colorado. Present during the inspection was Mr. <br />Jason McGraw of General Shale. <br />2) From the year 2000 thru 2008 a total of 204 acres were released under acreage reduction requests # 1 <br />thru # 3. The current permit acreage is 210 acres. The current affected acres at the time of the <br />inspection were approximately 9.0 acres. In 2008, the Division adjusted the reclamation warranty to <br />$36,314.00 for approximately 9.0 acres of disturbance. The disturbance includes excavation, <br />overburden, processed clay and topsoil stockpile areas and access road. No additional acres were <br />affected since that time. <br />3) Permit sign and boundary markers were properly posted. Some processed stockpile clay was noted <br />along the west side of the access road. <br />4) The entire affected area was walked and inspected. No major problem noted. All the drainage from the <br />affected area is captured with in the pit. No noxious weed infestation was observed at the time of the <br />inspection. The post mining land use for the site is rangeland. <br />5) The site is in compliance with the approved reclamation plan. <br />Page 2 of 4 <br />
