November 14, 2012 C- 2009 - 087 /Peabody Sage Creek Mine JDM <br />Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6; Discharge Structures 4.05.7, 4.05.10; <br />Diversions 4.05.4, Effluent Limits 4.05.2, Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13; <br />Drainage — Acid and Toxic Materials 4.05.8; Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br />Pond 004 - Pond 004 is a permanent impoundment originally permitted under C- 1980 -005, it was holding clear <br />water at the time of the inspection. Pond 004 utilizes an 8" smooth plastic pipe primary spillway and a rock lined <br />emergency spillway. The spillway was functioning as permitted. The emergency spillway appeared to be stable <br />with no problems noted at the time of the inspection. During the October 3, 2012 partial inspection the following <br />maintenance items were noted: <br />- A small erosion feature was identified at the toe of the emergency spillway. This erosion feature has been <br />repaired. <br />- It was noted that the outlet of the spillway of the original design of Pond 004 contained a small amount of water <br />in the culvert and a small amount of water was ponding at the outlet of the culvert. The inlet of the culvert was not <br />located and may have been capped upon approval of the current spillway design. The original culvert has been <br />removed and no pending was noted. <br />It is noted that the 8" primary outlet pipe has been replaced. The design criteria have not changed, as such, no <br />permitting action was required.
