The permit should be renewed for an additional term of five years in the name of the current <br />permittee, as follows: <br />Attached, consistent with the applicable requirements for permit renewal of Rule 2.08.5(2)(b), are the following <br />supplemental materials: <br />Copy of proposed Public Notice, consistent with applicable requirements of Rule 2.07.3(2) <br />Copy of current liability insurance policy (renewed annually each year during the term of the active <br />mining permit) <br />We appreciate your consideration, cooperation, and assistance in facilitating renewal of our active Mining and <br />Reclamation Permit, and look forward to working closely with you to coordinate on review and resolution of <br />any renewal issues.
