The above - mentioned site has one or more problems noted that, according to Division of Reclamation, Mining
and Safety (Division) records have not yet been resolved. According to our records, the following problems were
to be resolved on or before October 17, 2012. Listed below are descriptions and corrective actions for the
problems. Please resolve the corrective actions and provide evidence of such resolution on or before
December 6, 2012.
Please note that if the corrective actions are not properly addressed, and evidence provided to the Division by
the date provided above, the listed problems could be escalated to the level of possible violations and would
then be referred to the Mined Land Reclamation Board for further action.
PROBLEM /POSSIBLE VIOLATION:
The mine identification sign and affected area boundary markers were not observed per the
requirements of Rule 3.1.12.
CORRECTIVE ACTIONS:
The Operator must post a sign at the entrance to the mine site which shall be clearly visible from
the access road and specify the following; the name of the Operator, indicate that a reclamation
permit for the operation has been issued by the Colorado Mined Land Reclamation Board, and
the permit number. The boundaries of the affected area must be marked by monuments or
other markers that are clearly visible and adequate to delineate such boundaries. Photographic
evidence of the corrective actions must be submitted to the Division.
