Hay <br />Environmental Protection Specialist <br />Office of <br />Mined Land Reclamation <br />STATE OF COLORADO <br />Re: Willits Company, Inc.; McEndaffer Mine; Permit Number M- 2010 -034 <br />Follow -up on Unresolved Problems from September 13, 2012 inspection report <br />COLORADO <br />DIVISION OF <br />RECLAMATION <br />MINING <br />— &— <br />SAFETY <br />John Hickenlooper <br />Governor <br />Mike King <br />Executive Director <br />Loretta Pineda <br />Director <br />The above - mentioned site has one or more problems noted that, according to Division of Reclamation, Mining <br />and Safety (Division) records have not yet been resolved. According to our records, the following problems were <br />to be resolved on or before October 17, 2012. Listed below are descriptions and corrective actions for the <br />problems. Please resolve the corrective actions and provide evidence of such resolution on or before <br />December 6, 2012. <br />Please note that if the corrective actions are not properly addressed, and evidence provided to the Division by <br />the date provided above, the listed problems could be escalated to the level of possible violations and would <br />then be referred to the Mined Land Reclamation Board for further action. <br />PROBLEM /POSSIBLE VIOLATION: <br />The mine identification sign and affected area boundary markers were not observed per the <br />requirements of Rule 3.1.12. <br />CORRECTIVE ACTIONS: <br />The Operator must post a sign at the entrance to the mine site which shall be clearly visible from <br />the access road and specify the following; the name of the Operator, indicate that a reclamation <br />permit for the operation has been issued by the Colorado Mined Land Reclamation Board, and <br />the permit number. The boundaries of the affected area must be marked by monuments or <br />other markers that are clearly visible and adequate to delineate such boundaries. Photographic <br />evidence of the corrective actions must be submitted to the Division. <br />If the problems have already been remedied, please provide evidence of their completion.
