October 23, 2012 <br />Division of Minerals and Geology <br />1313 Sherman St., Room 215 <br />Denver, CO 80203 <br />Dear Sir or Madame: <br />RE: Annual Sand and Gravel 112 Report for 2011 -12, and Notice of Intent to <br />Continue Mining Operations (Permit No. M -84 -168) <br />Enclosed please find the Williams Fork Mining Company 2011 -12 Sand and Gravel Operations <br />report form, map, and annual fee payment ($791). The sand and gravel operation was idle this <br />year, but we intend to resume mining operations in 2013. The enclosed revised Appendix C <br />map illustrates the surveyed acreage that has been reclaimed (32.65 acres) and the unreclaimed <br />acreage that has been mined (16.26 acres) or is simply associated disturbance (22.62 acres). <br />The projection for the 2012 -13 report period (October 26, 2012 through October 25, 2013) is <br />for approximately 1.0 acres of new disturbance in Mine Area 6. It is anticipated that <br />approximately 1.5 acres will be permanently reclaimed. <br />On October 22, I discussed the issue of temporary cessation with Russ Means and he suggested <br />that we send in a request for such.
